At this time, the Low Vision Works program is only available to Medicare-eligible patients and those who wish to pay privately. Managed care insurance plans do not offer coverage to the best of our knowledge.

To qualify for Medicare reimbursement, the patient must:

  1. Be eligible for Medicare home health. Please visit www.medicare.gov for more information.
  2. Have visual acuity of 20/70 or worse in the better seeing eye with magnification, or suffer from a field loss of 20% or more (field cut).

The Low Vision Works program is exclusively and conveniently delivered in your home, over the course of eight weeks.

The best way to determine eligibility is to contact a Low Vision Works program representative at 866-590-0011.

Your The Low Vision Works program representative can help you check your Medicare eligibility and determine if the Low Vision Works program can be of benefit to you. You will need to provide your Medicare number and the name and telephone number of your eye care doctor. Medicare does require a prescription from your primary or eye care physician authorizing the delivery of the Low Vision Works program. We will obtain this prescription directly from your physician once it is clear that you are eligible
